Stage 3 Kidney Disease Creatinine Levels - (2023)

How High Creatinine Can Damage Kidneys

Kidney Disease Can Be Beaten! From Stage 5 to Stage 3 (Success Story)

Creatinine levels that reach 2.0 or more in babies and 5.0 or more in adults may indicate severe kidney impairment. There are many possible causes of high creatinine, some of which may be a one-time occurrence. Examples can include things such as dehydration or intake of large amounts of protein or the supplement creatine. All of these can be temporarily hard on your kidneys.

Shortness Of Breath After Very Little Effort

Why this happens:

Being short of breath can be related to the kidneys in two ways. First, extra fluid in the body can build up in the lungs. And second, anemia can leave your body oxygen-starved and short of breath.

What patients said:

At the times when I get the shortness of breath, its alarming to me. It just fears me. I think maybe I might fall or something so I usually go sit down for awhile.

I couldnt sleep at night. I couldnt catch my breath, like I was drowning or something. And, the bloating, cant breathe, cant walk anywhere. It was bad.

End Stage Renal Disease / Esrd

End stage renal disease is the final stage of chronic kidney disease, in which the kidneys are functioning at less than 15 percent of their normal capabilities. The kidneys experience complete or near complete failure and are unable to function on their own. It is most often caused by diabetes, but may also be a result of high blood pressure, vascular disease, an autoimmune disease or a genetic disorder.

End stage renal disease causes weight loss, nausea and vomiting, fatigue, difficulty breathing and seizures. If left untreated, end stage renal disease is a fatal condition.

At this stage, dialysis or a kidney transplant is usually needed. Patients will have to undergo dialysis treatments several times a week and may become very weak and fragile. Certain dietary changes may be necessary during dialysis treatment, including limiting fluids and salt and maintaining a low-protein diet. Transplants can often help restore patients’ health, but have long waiting lists and require daily supplemental medications as well. We encourage patients with advanced Chronic Kidney Disease to pursue transplant evaluation early in their disease to ensure that those fit to undergo kidney transplant can have a good chance of finding a kidney prior to ESRD.

What Are The Five Stages Of Kidney Disease

If youre newly diagnosed with chronic kidney disease , you may already know that there are five progressive stages, and related symptoms increase in severity along with the stage. You may not know, though, the ins and outs of each stage and what each stage means for you in a practical sense. So, what are the five stages of chronic kidney disease?

Risk Assessment Referral Criteria And Shared Care

Stage 3 Kidney Disease Creatinine Levels - (1)

Risk assessment

1.5.1 Give adults with CKD and their family members or carers information about their 5-year risk of needing renal replacement therapy .Follow NICE’s guideline on shared decision making when communicating risk.

1.5.2 Use every day, jargon-free language to communicate information on risk. If technical and medical terms are used, explain them clearly.

1.5.3 Set aside enough time during the consultation to give information on risk assessment and to answer any questions. Arrange another appointment for more discussion if this is needed.

1.5.4 Document the discussion on risk assessment and any decisions the person makes.

Referral criteria

1.5.5 Refer adults with CKD for specialist assessment if they have any of the following:

  • a 5-year risk of needing renal replacement therapy of greater than 5%

  • an ACR of 70 mg/mmol or more, unless known to be caused by diabetes and already appropriately treated

  • an ACR of more than 30 mg/mmol , together with haematuria

  • a sustained decrease in eGFR of 25% or more and a change in eGFR category within 12 months

  • a sustained decrease in eGFR of 15 ml/min/1.73 m2 or more per year

  • hypertension that remains poorly controlled despite the use of at least 4 antihypertensive medicines at therapeutic doses

  • known or suspected rare or genetic causes of CKD

  • suspected renal artery stenosis.

1.5.6 Refer children and young people with CKD for specialist assessment if they have any of the following:

When To See A Doctor With Stage 3 Ckd

Its important to see a doctor right away if you experience any of the above symptoms. While certain symptoms arent exclusive to CKD, having any combination of these symptoms is concerning.

You should follow up with your doctor if youve previously been diagnosed with stage 1 or stage 2 CKD.

Still, its possible to not have any previous history of CKD before getting diagnosed with stage 3. This could be due to the fact that stages 1 and 2 dont typically cause any noticeable symptoms.

To diagnose CKD stage 3, a doctor will conduct these tests:

  • blood pressure readings

Ckd Stage : Gfr Between 15 To 29

At stage 4 of chronic kidney disease, the patients kidney filtration capacity is reduced by a significant amount, causing accumulation of waste products in the blood leading to a disease called uremia. At this stage patient likely to develop other chronic diseases as well like cardiovascular disease, anemia, high blood pressure, bone disease, etc., and need to consult a nephrologist on every 3 months for routine checkup and monitoring. Here a nephrologist explains and educates further treatment options of dialysis or transplant to the patient to make a decision.

Symptoms: Kidney pain felt in the back, nerve problem , shortness of breath, fluid retention at extremities, nausea/ vomiting, bad breath, difficulty in concentrate, insomnia, restless legs, loss of appetite, etc.

Chronic Kidney Disease Stage 3 Creatinine Levels

Your doctor determines your stage of kidney disease based on the presence of kidney damage and your glomerular filtration rate , which is a measure of your level of kidney function. Your treatment is based on your stage of kidney disease. Speak to your doctor if you have any questions about your stage of kidney disease or your treatment.

Stages of Chronic Kidney Disease Stage: GFR* What it Means: Stage 1.

Your GFR is used to determine the stage of your kidney disease. Your serum creatinine level, which tells how well your kidneys are removing this waste product.

Keep your blood pressure levels under control. Consult a dietitian regarding useful changes in diet.

U.S. Task Force to consider routine kidney disease screening An influential U.S. panel has added screening for chronic kidney.

Screening for kidney disease typically consists of a blood test for levels of a waste product called creatinine and a urine.

Therapeutic Goal: Treat Proteinuria

Elevated Creatinine – Kidney Disease or False Positive? How to Differentiate

The first step in therapy is a protein-restricted renal diet. In addition, managing hypertension also helps minimize proteinuria. ACE inhibitors may cause hyperkalemia due to RAAS blockade, reduce GFR, and increase azotemia therefore, use these drugs cautiously in IRIS CKD stage 3 and 4 patients.

For persistent proteinuria, therapeutic intervention is recommended:

  • ACE inhibitor: Increase hypertension dosage to help minimize proteinuria however, contraindicated in hypotensive or dehydrated patients.
  • Omega 3-polyunsaturated fatty acids: Shown to lessen proteinuria.
  • Losartan: Consider this angiotensin receptor blocker for proteinuria refractory to ACE inhibitors veterinary use has been limited, with contradicting opinions regarding efficacy.

Anticoagulants can be considered when proteinuria is present however, serum albumin, UPC, or antithrombin levels do not adequately predict hypercoagulability.

What Does Stage 3 Kidney Disease Mean

At stage 3 CKD, your eGFR is between 30 and 59, which is a sign that your kidneys have some damage and are not working as well as they should be. Moreover, this stage of kidney disease is split into two by glomerular filtration rate :

  • Stage 3A: You have a GFR of 45-59 mL/min.
  • Stage 3B: You have a GFR of 30-44 mL/min.

When your kidney function declines, your kidneys are less able to filter waste products out of the blood. This can cause a build-up, called uremia. In stage 3 CKD, you also become more likely to have related complications, including:

Ckd Stage 3a And 3b: Gfr Between 45 To 59 And 30 To 44 Respectively

Stage 3 Kidney Disease Creatinine Levels - (2)

When eGFR drops between 30 to 59, kidney disease is considered to reach stage 3 where there is a significant reduction in the filtration of waste products by kidneys from blood. Stage 3 chronic kidney disease further classified into two that is stage 2A, when eGFR lies between 45 to 59, and stage 3B when eGFR lies between 30 to 44.

Symptoms: Shortness of breath, kidney pain felt in back, fluid retention, change in urine color , muscle cramp at night, insomnia, fatigue, and weakness, etc.

What Is A Kidney

The kidneys are a pair of bean-shaped organs on either side of your spine, below your ribs, and behind your belly. Each kidney is about 4 or 5 inches long, roughly the size of a large fist. Healthy kidneys filter about a half cup of blood every minute, removing wastes and extra water to make urine. The urine flows from the kidneys to the bladder through two thin tubes of muscle called ureters, one on each side of your bladder.

Stage 3 Chronic Kidney Disease

Stage 3 chronic kidney disease is considered the middle stage. In stage 3 chronic kidney disease, there is mild to moderate damage to the kidneys, and the kidneys dont filter waste and fluid as well as they should.

This waste and fluid may build up in the body and cause problems such as high blood pressure. Also, the decrease in viable kidney cells at this stage impairs their role in promoting healthy production of red blood cells, causing anemia, as well as their role in bone metabolism, causing problems in bone structure and metabolism which can lead to fractures.

While damage to the kidneys usually is not reversible, treatment and healthy lifestyle changes can slow the progression. With proper treatment, many people do not progress to stages 4 or 5, which is kidney failure.

Terms Used In This Guideline

This section defines terms that have been used in a particular way for this guideline. For other definitions see the NICE glossary.

Chronic kidney disease

Abnormalities of kidney function or structure present for more than 3 months, with implications for health. This includes all people with and those with a glomerular filtration rate of less than 60 ml/min/1.73 m2 on at least 2 occasions separated by a period of at least 90 days .

Classification of CKD

CKD is classified according to estimated GFR and albumin:creatinine ratio , using ‘G’ to denote the GFR category and ‘A’ for the ACR category , for example:

  • A person with an eGFR of 25 ml/min/1.73 m2 and an ACR of 15 mg/mmol has CKD G4A2.

  • A person with an eGFR of 50 ml/min/1.73 m2 and an ACR of 35 mg/mmol has CKD G3aA3.

  • An eGFR of less than 15 ml/min/1.73 m2 is referred to as kidney failure.

Creatinine Stages And Risks At Each Stage

How To Lower Your Creatinine Level Fast to avoid kidney failure and dialysis

Creatinine is a metabolic waste product produced by the metabolic activity of the body muscles. Creatinine is the determining factor to detect whether the kidneys are a function or not. The kidney functions to eliminate most of the creatinine out of the body via urine. When the kidneys are working well they efficiently excrete creatinine but high creatinine levels in the blood show chronic kidney disease .

CKD is a kidney disease that gradually shows its symptoms and is classified into 5 stages according to the severity of kidney failure . The kidney function is measured by Glomerular Filtration Rate . A mathematical formula involving the age, gender, race, and serum creatinine levels is used to calculate GFR. Blood tests are thus ordered to detect the stages of CKD.

Classification Of Ckd In Adults

1.2.1 Classify CKD in adults using a combination of GFR and ACR categories . Be aware that:

  • increased ACR is associated with increased risk of adverse outcomes

  • increased ACR and decreased GFR in combination multiply the risk of adverse outcomes.

1.2.2 Do not determine management of CKD solely by age.

Investigating the cause of CKD and determining the risk of adverse outcomes

1.2.3 Agree a plan to establish the cause of CKD during an informed discussion with the person with CKD, particularly if the cause may be treatable .

1.2.4 Use the person’s GFR and ACR categories to indicate their risk of adverse outcomes and discuss this with them.

Understanding Your Lab Values

Stage 3 Kidney Disease Creatinine Levels - (3)

People who develop chronic kidney disease may have some or all of the following tests and measurements. If you have kidney disease ask your doctor which tests you will have and how often they will be done. Speak to your doctor about your results. If your numbers are not in the normal range, ask how to improve them.

Serum Creatinine: Creatinine is a waste product in your blood that comes from muscle activity. It is normally removed from your blood by your kidneys, but when kidney function slows down, the creatinine level rises. Your doctor should use the results of your serum creatinine test to calculate your GFR.

Glomerular Filtration Rate : Your GFR tells how much kidney function you have. It may be estimated from your blood level of creatinine. If your GFR falls below 30 you will need to see a kidney disease specialist . A GFR below 15 indicates that you need to start a treatment for kidney failure. Your kidney disease specialist will speak to you about treatments for kidney failure, such as dialysis or kidney transplant.

Blood Urea Nitrogen : Urea nitrogen is a normal waste product in your blood that comes from the breakdown of protein from the foods you eat and from your body metabolism. It is normally removed from your blood by your kidneys, but when kidney function slows down, the BUN level rises. BUN can also rise if you eat more protein, and it can fall if you eat less protein.

COVID-19 patients can become kidney patients.
